Membership Agreement
Effective Date: March 1, 2026
This Membership Agreement outlines the terms governing your participation in Rural Health Net™, a Discount Health Care Program.
1. Program Description
Rural Health Net™ is a Discount Health Care Program. It is not insurance and does not pay for or reimburse the cost of health care services.
2. Membership Eligibility
Members must be at least 18 years old. Minors may be included only when enrolled by a parent or legal guardian.
3. Member Obligations
- Pay membership fees as scheduled
- Pay providers directly at the time of service
- Present valid membership information to receive discounts
4. Provider Network
Providers are independent and may change at any time. Discounts vary by provider and service.
5. Fees, Billing, and Cancellation
Membership fees are billed on a recurring monthly basis for a single plan. You authorize Rural Health Net™ to charge your selected payment method for membership fees until you cancel.
You may cancel your membership at any time by calling our toll-free Member Support number at 888-860-1874 or by submitting a written cancellation request through email at info@ruralhealthnet.com.
If you cancel within the first 30 days of your initial enrollment and have not used any discounted services, you will receive a full refund of all membership fees paid. After the first 30 days, your membership will remain active through the end of your current billing period, and no further charges will occur.
Refunds, when applicable, will be issued to the original payment method within 7–10 business days.
This program is not insurance. Membership fees provide access to discounted services only and are not payments for medical care.
6. Limitation of Liability
Rural Health Net™ is not responsible for provider actions or medical outcomes.
Our total liability is limited to the amount of membership fees you have paid in the twelve (12) months prior to any claim whether based in contract, tort, negligence, strict liability, or otherwise.
